Type 2 Diabetes Indigenous and CALD Access Project

Purpose

To identify high risk groups for diabetes in the IEPCP and identify strategies to support
increased access for high risk groups to services involved in the Eastern Metropolitan
Improving the Journey - Type 2 Diabetes System Improvement Project.

Introduction

In broadening the scope of the ITJ project to take into account the Aboriginal and Torres
Strait Islander (ATSI) and CALD communities in the region the applicability of Agreed
Packages of Care processes and resources to the Aboriginal and Torres Strait Islander
(ATSI) and CALD communities will be explored.

Methodology

The methodology proposed involves a four staged approach;

Stage One: Identifying high risk groups in the catchment

A review of local demographic data and diabetes prevalence data to identify high risk groups across the catchment. 

Stage Two: Identifying local service utilisation patterns

A map of current diabetes services targeted at high risk groups in the catchment and service utilisation of identified high risk groups. 

Stage Three: Identifying access and engagement strategies

A literature review was completed to identify strategies for engaging with and improving access to services for identified high risk groups.

Stage Four: Developing strategies to improve access to services for high risk groups

Work will be conducted with local agencies to identify service gaps for at risk groups, jointly plan programs to address gaps and look at consistency of care across services to support evidence based practice for identified high risk groups.
