2001census - cultural analysis
Demographics of the Eastern Region
ABS Census 2001 The ABS Census 2001 found that 20% (186,963) people living in the Eastern Region speak a language other than English at home. One hundred and fifty one different langauges are spoken across the Region by people born in over 190 different countries. The largest number of people who speak a language other than English at home speak Greek (33,290) followed by Cantonese (27908), Italian (24948), Mandarin (17493), German (6206) Vietnamese (5816) and Arabic (5145). |

2008 Service Coordination/ICDM Survey

The standard of health of our Aboriginal and Torres Strait Islander population is
considerably poor by comparison with other Australians. Although improvements have been made in recent years the gap in life expectancy for Aboriginal and Torres Strait Islander peoples compared to the rest of the population remains significantly high and in marked contrast to other comparable countries. Many causes of ill health and premature death are preventable, yet a number of barriers continue to restrict Aboriginal and Torres Strait Islander peoples’ access to an equitable, quality health system. |

Boroondara Small Planned Activity Groups (Service Coordination)
This report describes an attempt to integrate into the Inner East Primary Care Partnership service coordination approach a number of small Planned Activity Groups in the City of Boroondara. This was done by introducing them, as participating agencies, to the Eastern Metropolitan Region electronic service coordination system (e-referral or E-SCS).
Involvment in e-referral proved to be an effective approach to developing greater awareness of the health sector service system. However, developing guidelines for assisted referrals, promoting the importance of providing feedback for referrals received and fostering a referral mindset amongst Planned Activity Groups would achieve even further benefits. |

Care Planning Report
This report sets out the deliberations, activities and recommendations of the IE PCP Care Planning Working Group. Consumer Involvement in Member Agencies
The agency self assessment audit of consumer participation across the CE PCP catchment was undertaken for two reasons. Firstly, the audit provides a comprehensive picture of the nature and level of consumer participation already occurring within CE PCP agencies. From this we can draw attention to examples of ‘good practice’ that can serve as an illustration of what can be achieved. The second reason for the audit was to establish a starting point for ongoing work with member agencies in the development of consumer participation within their services.

Consumer experiences of treatment for depression: pathways to recovery
This research was made possible by the partnership between Deakin University, the Inner East PCP and the Primary Mental Health Team (Eastern Health). The project was funded with a seeding grant from beyondblue, the national depression initiative. The Victorian Centre for Excellence in Depression and Related Disorders. The School of Health and Social Development, Deakin University provided additional funds.

Consumer experiences of treatment for depression
This project involved Deakin University, The Central East PCP, Eastern Healths Primary Mental Health Team and other primary care providers. Funding was provided by Beyond Blue. The project aimed to study how people experienced mental health care in the Eastern Region specifically with a focus on 'shared care' and was concerned with how social determinants shaped their care. 42 consumers experiencing depression and anxiety were interviewed.

EMR Care Planning/Care Coordination Initiative (CPCCI)
Wednesday 28 November was a significant day for the EMR Care Planning/Care Coordination Initiative (CPCCI) as the second Steering Committee meeting was held in Box Hill Town Hall Hub. Over fifty people form the Inner and Outer PCPs attended and the program was very full and provoked a lot of interest. The attached minutes give a good idea of what transpired and the attached presentations give a clear idea of material on the agenda.
It was decided at the Steering Committee meeting to hold an intensive working party to put together an Eastern Metropolitan Region view of what revisions should be made to the Service Coordination Plan. This met on Tuesday 4 December and the outcome of that meeting will be distributed shortly to the CPCCI Consultative Group. Thanks to all those who contributed to the initiative. It is a difficult task but we do seem to be making good progress and will keep you posted throught the Consultative Group on what will be happening in 2008. |

Fall Injury Prevention Resource Kit - Facts on falls among seniors
A kit to provide health and care professionals with current technical information on the evidence base for best practice in the prevention of falls and fall-related injuries in older people as well as access to resources.
The kit contains the following fact sheets: 1. How serious is the problem? 2. What are the risk factors? 3. What can be done? Summary of research evidence 4. Exercise programs: research evidence base 5. Individual falls risk assessment and abatement: research evidence base 6. Home & public place risk assessment and reduction: research evidence base 7.
