E-SCS workshop
A workshop will be held on Tuesday 22 April with the main aims of:
- reporting back on the survey conducted in 2007 by Ocean Informatics;
- considering how the recommendations in the Ocean Informatics report might be implemented;
- getting an understanding of how the various IT initiatives (such as Trakcare, Healthsmart etc.) fit together;

Improving the Journey: Agreed Packages of Care
This resource provides the framework for the regional EMR Improving the Journey Agreed Packages of Care for people with Type 2 Diabetes.

The agreed packages of care outline baseline packages which agencies have agreed to support when working with people with type 2 diabetes. Services within the packages of care may be provided across multiple agencies.

Agencies are able to add to these packages (eg. if additional services are available). At a minimum, consumers have the opportunity to access the services outlined within the relevant agreed package of care.

EMR Care Planning/Care Coordination Initiative (CPCCI)
Wednesday 28 November was a significant day for the EMR Care Planning/Care Coordination Initiative (CPCCI) as the second Steering Committee meeting was held in Box Hill Town Hall Hub. Over fifty people form the Inner and Outer PCPs attended and the program was very full and provoked a lot of interest. The attached minutes give a good idea of what transpired and the attached presentations give a clear idea of material on the agenda.

It was decided at the Steering Committee meeting to hold an intensive working party to put together an Eastern Metropolitan Region view of what revisions should be made to the Service Coordination Plan. This met on Tuesday 4 December and the outcome of that meeting will be distributed shortly to the CPCCI Consultative Group.

Thanks to all those who contributed to the initiative. It is a difficult task but we do seem to be making good progress and will keep you posted throught the Consultative Group on what will be happening in 2008.

The HACC/CALD Access and Support Pilot Project
The Access and Support Pilot Project focused on the culturally and linguistically diverse (CALD) population eligible to use Home and Community Care (HACC) services and alleviating difficulties experienced in the past by such clients in firstly accessing HACC services and then remaining engaged with those services.

The purpose of the project was to develop and pilot a model for providing access and support services for CALD clients to improve access to and outcomes of Home and Community Care services.

Overall, the project was successful in meeting the objectives with the access and support requirements of CALD communities defined and a model developed that outlined the roles and responsibilities of organisations involved and protocols and pathways for the provision of access and support.'

MonashLink launches Early Intervention in Chronic Disease program
On 15th October, Gregg Nicholls CEO of MonashLink Community Health Service, launched MonashLink’s Early Intervention in Chronic Disease (EIiCD) program. Invited guests and MonashLink EIiCD partners gathered at Monash Gallery of Art to hear about and celebrate this program which aims to further support the health of people with chronic disease in our region.

MonashLink EIiCD is the result of collaboration between MonashLink and Greater South Eastern Division of General Practice and is supported by the Inner East Primary Care Partnership.

The Early Intervention in Chronic Disease initiative is funded by Department of Human Services to assist community health services to build on the work already being done to support people in the community who have chronic disease. It provides additional funding to increase service delivery and build new components that are consistent with evidence-based chronic care (ie. care coordination and self management coaching &/or support for clients).

MonashLink is one of eighteen (18) community health services in Victoria to have received this funding to date. Another EIiCD program within our catchment, the Good Life Club at Whitehorse Community Health Service, has been in operation since 2006.

Social Connectedness & Health - 'A Literature Review'

Social Connectedness Project
In the mid 1980’s the World Health Organisation promoted a more holistic approach to health through the Ottowa Charter for Health Promotion. Boroondara Small Planned Activity Groups (Service Coordination)
This report describes an attempt to integrate into the Inner East Primary Care Partnership service coordination approach a number of small Planned Activity Groups in the City of Boroondara. This was done by introducing them, as participating agencies, to the Eastern Metropolitan Region electronic service coordination system (e-referral or E-SCS).

Involvment in e-referral proved to be an effective approach to developing greater awareness of the health sector service system. However, developing guidelines for assisted referrals, promoting the importance of providing feedback for referrals received and fostering a referral mindset amongst Planned Activity Groups would achieve even further benefits.

Memorandum of Understanding - July 2006-June 2009
This Memorandum of Understanding (MOU) has built on past MOUs from the last 5 years and now lays the framework for the work of the Inner East Primary Care Partnership (IEPCP) for 2006/09. Over the past five years member agencies have invested time and expertise into multi sector partnership work that has been focused on integration of services and health promotion. In 2004 two PCPs (Central East and Boroondara) aligned their efforts and created one PCP that is called the Inner East PCP with a population of approximately 590,000 people.

Regional Electronic Service Coordination Agency Responsibilities
The introduction of the Eastern Region Electronic Service Coordination System (E-SCS) is an essential element of the Inner East PCP and OEHSCA vision to facilitate better access and care outcomes for consumers by improving service coordination and increasing consistency in practice between member agencies.
