- Events, calendar and meeting schedule 2012. This item shows the service coordination calendar for 2012 and includes dates for Practitioners' Network meetings and e-referral training.
- Service Coordination PPPS Template/Implementation Guide. This resource is designed to assist agencies draw up policies and procedures on the implementation of service coordination PPPS. It also serves as a good introduction for agencies new to service coordination.
- Service Coordination 2011-13 Workplan. This document outlines initiatives in this period for the further development of Service Coordination and e-referral.
- Service Coordination Manuals. This website contains the 2009 Victorian Service Coordination Manual, the 2009 Good Practice Guide for Practitioners and the 2009 Continuous Improvement Framework.

- Service Coordination Advisory Group Terms of Reference. The Terms of Reference for the Service Coordination Advisory Group, the body that oversees service coordination activities within the IEPCP at a strategic level.
- Service Coordination Practitioners' Network Charter. This is the Charter for the IEPCP Service Coordination Practitioners' Network, which is a community of practice with an annual program that focuses on service coordination issues of interest to IEPCP agencies.

- 2010 Service Coordination & ICDM Survey results. This link provides an overview of State, regional and IEPCP results. Participating agencies have received their own results together with an analysis by the IEPCP.
- Service Coordination Online Learning Module. This is the most recent draft of the online learning module that will replace the 2009 Self Paced Training Module - the new version should be available round mid-2012 and is intended to be used as part of the orientation process to service coordination for new staff. - Events, calendar and meeting schedule 2012. This item shows the service coordination calendar for 2012 and includes dates for Practitioners' Network meetings and e-referral training.
- s2s User Manual This manual contains a detailed description of the Electronic Service Coordination System.
- Electronic Aged Care Packages Waitlist System Manual This manual contains a description and instructions for the waitlist system.
- Merge Client Quick Guide This function enables users to merge clients. It is for use when a client's information is being kept under two (or more) separate identities (maybe arising from aliases, maybe typing errors, maybe just plain misunderstanding). Instructions are contained in the Quick Guide.
- Registration Procedures (general). These procedures will allow Inner East PCP agencies to become registered users of the Electronic Service Coordination System.
- Individual Registration procedures. New users should follow the Individual Registration procedures very carefully and complete the registration form as instructed. However, authorisation by the user's manager is still required before the system can be used (see below).
- Authorisation to use the system (for Managers). This form must be completed each time there is a change to the users within your organisation e.g. a user is added, deleted or has changed details. The completed form must be faxed to Infoxchange.
- Eastern Metropolitan Electronic Service Coordination System website. You must be a registered and approved user to gain full access to the system.
- E-SCS/s2s demonstration site. This section contains a cheat sheet to learn to use E-SCS/s2s (including the URL). You do not need to be a registered user to use the demonstration system.
- Super User Guide. This guide is for the more advanced user, who will often act as a trainer to others.
- Waitlist site. Use this link to access the CACPs/EACH Waitlist system, which allows for the matching of consumers with aged care packages. You must be a registered user to access this system
- Waitlist demonstration site. This web address will take you to a demonstration site on which you can learn to use the Waitlist system. You do not need to be a registered user to use the demonstration system.
- e-referral roles and responsibilities. This table shows the various roles within the e-referral system and the associated functions.
- Agency policies and procedures template. This template is to assist agencies to draw up their own set of e-referral policies and procedures. Much of the content can be pasted into your documentation.
- RDNS referral procedures. This document sets out procedures for making referrals to RDNS and receiving referrals from RDNS.
- GP referral procedures. This document contains procedures for referrals made to an from GP practices and the wider service sector using the Argus messaging system, s2s and GP software.
- Written Referral Guidelines A guide on when to make formal, written referrals.
- Benefits of e-referral. This document outlines the benefits from using e-referral and, in particular, from fully involving practitioners in e-referral (rather that confining e-referral management to Intake Workers).

Hoarding workshop - November 2011
The IEPCP held a workshop on hoarding behaviour on 21 November. Major outcomes were a greater understanding of the nature, ramification and (very high) incidence of hoarding behaviour and a clear indication that it should be regarded as a psychological/psychiatric problem. Presenters were Julie Harris, Community Ageing Strategist with the Metropolitan Fire Board and Dr. Christopher Mogan, a psychologist and specialist in hoarding behaviour.

e-referral workshop 8 December 2011
An e-referral workshop was held on 8 December 2011 for IEPCP members. The agenda included presentations on Personally Controlled Electronic Health Records (PCEHR) by Adam McLeod of Inner East Melbourne Medicare Local, on electronic care coordination by Amodha Ratnayeke of Infoxchange and Vivian Blacker of Creative Synthesis and on e-referral performance and on gaps and possibilities presented by Christopher Foley-Jones of the IEPCP. The workshop then prioritised issues, the results of which can be seen in this table.
The key messages were that while implementation of e-referral in the IEPCP has been good, there is still a long way to go before reaching anything like what is possible. The electronic care coordination module has progressed to a point where the IEPCP should be considering how to implement it (probably in early 2012). Finally, the health sector would be well advised to work out now how local systems and practices are going to fit in with developments at a Federal level, with particular reference to PCEHR. Even though initial functionality will be limited, PCEHR is due to go live in our catchment in June 2012. By then all individuals should have the required ID number and current understanding is that PBS data and some other functions will also be available.

WWWT Pilot - City of Monash
In conjunction with bestchance Child Family Care, City of Monash Maternal & Child Health and Jean Hailes Research, Monash University, the IEPCP facilitated a pilot program in the City of Monash in 2011 to test out the practicability of the What We We Thinking treatment program. This program, developed by Jean Hailes Research, is designed to prevent postnatal depression through a short term course involving couples and their baby. The program has a primary focus on infant sleeping patterns and interpersonal relationships as these have been found, by Jean Hailes Research, to be major causal factors in postnatal depression.
The success of the pilot contributed to Jean Hailes Research being awarded a grant of $700,000 to implement the program in a further six local government authorities in Victoria.
