This page outlines current areas of service coordination activity and projects within the Inner East Primary Care Partnership.

IEPCP Strategic Directions 2009-12

Service Coordination activities are governed by an overarching Strategic Directions Plan covering the period 2009-12 and focusing on three priority areas – ageing, chronic conditions and mental health and well-being.

Service Coordination Activities 2011

Service Coordination has an ongoing program including the following elements:

  • Contributing to the development of Statewide service coordination practices, processes, protocols & systems  (PPPS).  Currently these activities include reviews of the Victorian Service Coordination Practice Manual, Good Practice Guide for Practitioners, Continuous Improvement Framework, the Service Coordination Tool Templates and the Self Paced Training Module.
  • Implementation of service coordination PPPS in the Inner East catchment
  • Implementation of e-referral (Electronic Service Coordination System) in the Inner East catchment
  • e-referral and service coordination training courses - see Service Coordination Calendar
  • Developing and maintaining a community of practice amongst agencies that use service coordination - major mechanisms for achieving this are the Practitioners' Resource Group and the Service Coordination Steering Committee
  • The Service Coordination Steering Committee has under it two Working Parties - one dealing with Service Coordination practices, processes, protocols & systems, the other with e-referral

  • Meeting the requirement of the Department of Health program logic
  • Other projects as decided from time to time.

In the course of the year there will be regular feature presentations at the monthly Practitioners' Resource Group meetings and a number of forums will be held.  For dates and events, please see the Service Coordination Calendar.

E-Referral Activities 2011

Priority action areas for the further development and implementation of the Electronic Service Coordination System are:
 
This project uses the Argus messaging system to link GP software packages and the E-SCS/s2s e-referral system. The Victorian Statewide Referral Form is auto-populated by the GP software (e.g. Medical Director) and is then transmitted via Argus to the E-SCS/s2s system, where it is integrated into the database. The reverse procedure has also been developed whereby referrals made to GPs by the public health sector agencies auto-populate client files within the GP clinic.

A project with a high priority is the inter-connection of the Eastern Metropolitan Region's Electronic Service Coordination System (E-SCS) database and the s2s database used elsewhere in Victoria.  Currently e-referral is restricted to agencies within one or the other database and an inter-connection should substantially increase the reach of both systems.
 
Interoperability between the various kinds of software used in the health and human services sector is also a high priority.  The area in which this most impacts is the transfer of data between the e-referral system and the various client management software packages.  The goal is to implement HL7 healthcare interoperability standards to enable the various systems to communicate seamlessly without double data entry.
