PCP Membership
Agencies involved in primary care, health, support and social services and who deliver those services within the four local Government areas of Boroondara, Manningham, Monash and Whitehorse, are invited to join the Inner East Primary Care Partnership.
Agencies wishing to join the Partnership are asked to complete the Membership Form and sign the Memorandum of Understanding (MOU). The Memorandum of Understanding sets out the basis of the Partnership, the Governance and Committee Structure and the overall purpose and aims.
