Introduction to Improving the Diabetes Journey
The Improving the Diabetes Journey project is a systems-level change project, which supports local partnering agencies to enact change to improve care for people with chronic disease (with a specific focus on type 2 diabetes).
Project goal
To improve the health and wellbeing of people with or at risk of type 2 diabetes, through the development of a regional approach to the prevention and management of type 2 diabetes.
Project principles
Improving the Journey is built on the following key principles:
- Consumers at the centre
- Equity of access
- Focus on populations at risk
- Evidence based systems and services, and;
- A whole of system (redesign) approach.
Project outcomes
Predicted outcomes over 4 year project:
- Improved consumer experience of coordinated care
- Improved processes to support prevention, early identification and intervention for people at risk of type 2 diabetes and its complications
- Improved awareness of services available, by consumers and clinicians
- Development of processes which support coordinated care across services and disease stages
- Increase in integrated processes for prevention and management.
