Mapping of current diabetes services targeted at high risk groups in the catchment and service utilisation of identified high risk groups. - Stage 3 Report: Identifying access and engagement strategies
A review of literature which identifies strategies for engaging with and improving access to services for identified high risk groups. - Stage 4 Report: Developing strategies to improve access to services for high risk groups
This report will utilise the information collected in stages 1-3 and work with local agencies to identify service gaps for at risk groups and strategies to address gaps.
