Improving the Diabetes Journey
The Improving the Diabetes Journey project is a systems-level change project, which supports local partnering agencies to enact change to improve care for people with chronic disease (with a specific focus on type 2 diabetes). Click here for more information.

Framework
A framework for service delivery for people with type 2 diabetes in the Eastern Metropolitan Region (EMR) has been developed. The framework outlines the services that all consumers with type 2 diabetes in the EMR must have the opportunity to access, at different life or disease stages. This framework is referred to as the ‘Type 2 Diabetes Care Pathway’. Click here for the 'Implementation Guide' for more information.

Type 2 Diabetes Indigenous and CALD Access Project
The purpose of this project is to identify high risk groups for diabetes in the IEPCP and identify strategies to support increased access for high risk groups to services involved in the Eastern Metropolitan Improving the Journey - Type 2 Diabetes System Improvement Project.
