Goal-directed care planning – training courses
Applications are being taken for the competency unit CHCCM702A - Implement goal-directed care planning. The program is particularly relevant for HACC assessment officers, case managers and allied health professionals currently involved in care planning. Applications are being taken on line at: www.health.vic.gov.au/pcps/goaldirected.htm until March 2012.

IHP Training Needs Analysis-Outcomes Report
In 2011, Inner and Outer East PCP surveyed health promotion practitioners working in the EMR to identify and prioritise their training needs. The outcomes of this Training Needs Analysis will be used to inform capacity building activities for HP practitioners in 2012. A copy of the TNA Outcomes Report for the Inner East region is available on our website.
E-referral Training
Training dates for centralised e-referral training (for beginners and those who have forgotten all they ever learned) can be found on Events, calendar and meeting schedule 2012.
Courses are in the Box Hill Town Hall Computer Training Room from 9-12 on one Friday each month. Enquiries to christopher.foley-jones@iepcp.org.au
Improving the Diabetes Journey-2012 workshops are starting!
The Improving the Diabetes Journey project is a systems-level change project, which supports local partnering agencies to enact change to improve care for people with type 2 diabetes. The four collaborative workshops in 2012 focus on system change and aim to support agencies to plan and implement changes to support the delivery and use of the:
• Type 2 Diabetes Care Pathway
• The Type 2 Diabetes Priority Pathway Tool
• The Client Information Pack
• Recall and Reminder Map
• A GP Communication Plan

Service Coordination Advisory Group
What was previously the Service Coordination Steering Committee has been re-named and has a somewhat tighter focus. Refer to Service Coordination Advisory Group Terms of Reference for details. This group comprises solely agency Service Coordination Contacts and proxies and operates at a strategic level on matters associated with service coordination.

SAVE THE DATE: Cinema Fundraiser Event for the Opening Doors Program-March 14th
You are invited to attend a Cinema fundraising event for the Opening Doors Program on 14th March at the Palace Cinema in Balwyn (starting time TBC 6-7pm).
The movie is ‘EXTREMELY LOUD AND INCREDIBLY CLOSE’ and is based on the acclaimed novel of the same name. It tells the story of one young boy's journey from heartbreaking loss to the healing power of self-discovery, set against the backdrop of the tragic events of September 11.

Evaluation report: 'Go for your life' is now available
In 2006, six 'Go for your life' community demonstration projects were funded. The overall aim of the projects was to increase healthy eating and physical activity levels in children, families and adults in areas of socio-economic disadvantage. Five Primary Care Partnerships (PCPs) and one Victorian Aboriginal Community Controlled Health Organisation (VACCHO) were funded. These projects were completed in June 2010 with the final evaluation recently completed and made available online.
