IEPCP 'Planning for the Future' Forum
The IEPCP will be holding a special forum on Thursday 25 August 2011, 9 a.m. to 12.30pm in the Whitehorse Room, ground floor, Box Hill Town Hall.
This is an opportunity for partners of the IEPCP to re-visit the benefits of collaboration in primary care and review the progress of our partnership work to date. You will hear about transition plans for Inner East Melbourne Medicare Local, & have an opportunity to assist in identifying local health and wellbeing priorities for the Inner East 2011-12 and onwards.
Who should attend?
This forum is for all service providers, managers, program coordinators, project workers and practitioners working in partnership with Inner East PCP. For a detailed program, please click here.

ADMA Conference
The IEPCP is presenting at the Partnerships for Chronic Care Patients, Services, Policy Conference on the 25-26th August 2011. The IEPCP will be presenting two interactive poster discussions on the Type 2 Diabetes Indigenous and CALD Access Project and the Improving the Diabetes Journey - A Type 2 Diabetes System Change Project.

Service Coordination and ICDM Statewide Survey
The 2011 Service Coordination & ICDM survey will be going on-line (for a month) on 1st September. The IEPCP is holding a briefing session on Wedesday 17th August, following directly after the Service Coordination Steering Committee meeting. Type 2 Diabetes Indigenous and CALD Access Project

Active and Inclusive Communities Kit
An 'Active and Inclusive Communities Kit' is now available for clubs and physical activities providers. It was developed by the Central Gippsland PCP, Healthy and Active Communities and Problem Gambling Initiative programs. They are happy for this resource to be adapted for use in other areas.

City of Monash leads State in training
HACC services at the City of Monash is leading the State in ensuring all their assessment team have appropriate formal qualifications. All staff have completed the Vocational Graduate Certificate in Assessment and Care Planning. This is a first in Victoria and Monash is to be congratulated on professionalisation of its workforce.

Dementia - diagnosis and management - special PRG session
The next Practitioners' Resource Group meeting will be held on Monday 22nd August 9.30-11.15. The meeting will feature Madeleine Adams of SACS Cognitive Services and Ben Gatehouse of Southern Cross Care, respectively, who will present on the diagnosis of dementia, early management and future planning and the use of music and other creative approaches in managing dementia.
