IEPCP Directions 2009-2012 - IEPCP Strategic Plan
IEPCP Directions 2009-2012 (revised June 2011) is the current IEPCP's strategic plan that articulated the vision, priority areas and strategic objectives for the partnership.
The plan is reviewed annually, and takes into account broad environmental variables, local social and population health needs, our existing service system and the potential directions suggested by current primary care reforms. The focus of our current collaborative work, as per revision process conducted in March/April 2011, expands on the three distinct yet inter-related priority areas initially identified for the 2009 - 2012 period.
IEPCP focuses on capacity building activities in partnership with member agencies in the Inner East metropolitan catchment. These activities align with one or more of the Key Domains of Activity that are the remit of Victorian PCPs:
- Partnerships
- Service Coordination
- Integrated Health Promotion
- Integrated Chronic Disease Management

Capacity building activities focus on benefitting those:
- who have complex and chronic conditions;
- who are ageing
- who experience poor mental health
- who experience social isolation
- who are at risk of, or have experienced family violence
- who are at risk of, or have, a gambling problem.
Partnerships with health, home and community care sectors within Manningham, Boroondarra, Whitehorse and Monash local government areas form the platform for system and practice reforms to improve the health and wellbeing of individuals and communities.

Strategic Directions 2012-13
The Department of Health have indicated their agreement to align PCP strategic planning cycles with local government Municipal Public Health Plans and other key sectors' strategic cycles. this requires PCPs to move from 3 year to 4 year cycles.
To support this transition, IEPCP will be developing a 'bridging' year plan for 2012-13 in consultation with key stakeholders. This plan will essentially review and confirm current priorities and identify new priorities to be addressed during the period of the plan. Implementation will be supported by IEPCP Implementation Plans in each of the four Domains of Activity.
After this time, a full strategic planning process will be conducted to identify strategic directions for the four year period 2013-17.
