ICDM Implementation Plan 2011-2012

This documents outlines the current objectives and strategies for Integrated Chronic Disease Management in the Inner East Primary Care Partnership Catchment.

The activities have been collaboratively developed and implemented by PCP ICDM partner agencies over a 1 year period.
