Type 2 Diabetes Indigenous and CALD Access Project
To identify high risk groups for diabetes in the IEPCP and identify strategies to support
increased access for high risk groups to services in the Eastern Metropolitan Region.
Department of Health Plan Do Study Act Model for Improvement Project
The Plan, Do, Study, Act (PDSA) Model for Improvement project supported State-funded primary health care agencies to increase their capacity to care for clients with complex needs. The project is based on the Collaborative Method to drive quality improvement through sharing of knowledge and experiences with others. Two agencies in the IEPCP catchment focused on improving communication with GPs by developing standardised feedback forms.

Abstract and Poster Presentation Skills Workshop
The 2 Abstract and Poster Presentation Skills Workshops were completed in Oct-Nov 2011. The workshops supported agencies in developing an abstract and a poster for conferences and other events. The following posters were produced on health projects in the Eastern Metropolitan Region:
