ICDM Education and Events Calendar
IEPCP Is committed to providing workforce development initiatives to the Inner East region. This calendar runs from June 2011- June 2012.
| Eligibility: | These workshops are for clinicians working with clients with chronic and complex conditions in the Eastern Metropolitan Region. |

Developmental Disability Health Workshops
IEPCP in conjunction with CDDHV are providing training on development disability. Aimed at community health services clinicians, intake works and other staff, it helps you improve you work with people with a developmental disability and the accommodation services they reside within.

2011 Service Coordination and ICDM Survey
The Statewide Service Coordination (SC) and Integrated Chronic Disease Management (ICDM) Survey provides agencies quantitative evidence of coordinated care for people with multiple or complex needs and provides evidence for agency accreditation and program quality assurance requirements. Conducting these surveys annually provides an opportunity to monitor changes that may have occurred as a result of improvement strategies.

Improving the Diabetes Journey
The Inner and Outer East PCPs are conducting a Type 2 Diabetes System Improvement Project, entitled Improving the Diabetes Journey, in the Eastern Metropolitan Region.

Workforce Innovations Grant- Strategic Workforce Planning Project
The project aims to develop a standardised Training Needs Analysis process and resource kit (the ‘Toolkit’) for use by PCPs across Victoria. This resource will support PCP’s in determining the learning needs of agencies’ workforce, thereby facilitating improved planning and delivery of training programs by PCPs across the state.

Assessing Chronic Illness Care in the IEPCP Catchment
This project supports agencies in the local government areas of Boroondara, Manningham, Whitehorse and Monash to identify areas for improvement for chronic illness care. This project runs from October 2011- June 2012.
