Primary Care & Health Networks across the Inner East Catchment

Below is a listing of mostly professional networks that exist across the four municipalities of the Inner East. This is not an exhaustive list and the identified networks relate to the priority areas of the Inner East PCP.
A key agency is provided as the contact and will be able to provide information on where, when and how often the network meets.

Health

Boroondara Health Promotion Network

To fulfill a leadership role in health promotion that drives the development of a strategic, collaborative and integrated approach. Organisations and services who are service providers within the City of Boroondara

A reference group for the Boroondara Falls Prevention project. Provides resource development, training & education, 4 week step safely program, awareness raising activities, develops a partnership approach

Boroondara Mental Health Services Group

Network of services who provide a variety of support for people with a mental health issue. Meets on the fourth Thursday of the month, 10 am to 11 am, at the Hawthorn Community Mental Health Centre, 642 Burwood Road, Hawthorn East - between Auburn Road and Camberwell Road.

Boroondara Volunteer Resource Centre Agency Network

Meets to provide information and support to volunteer managers and coordinators of its member agencies bi-monthly at the City of Boroondara offices in Camberwell, on a Wednesday morning (dates for 2005 TBC),.

Older Persons

Boroondara Aged Service Providers Association ( BASPA)

A network of 35 HACC funded agencies which seeks to strengthen the aged and communitycare service system with a planned, coordinated and integrated approach to enhancing the health, well-being and independence of the elders of
Boroondara. Meets every six weeks at the Elgin Street Centre, Elgin Street

Inner East Health Promotion Network

For representatives within and outside the health sector in the Inner East who have an interest in health promotion. Meets monthly (February - November) on the second Thursday of the month, from 2:30pm - 4:30pm Contact – Eastern Regional DHS Health Promotion Officer PH 9843-6698

Inner East Healthy Ageing/ Falls Prevention Steering Reference group

For those with an interest in prevention of falls for older people. This group works to plan, implement, evaluation, and provide skills advice and expertise to the Inner East Falls Prevention Project. Meets 10.30am-12.30pm on the first Thurs of the month.
