About Us

Introduction


The Inner East Primary Care Partnership (IEPCP) covers the local Government areas of Boroondara, Manningham, Monash and Whitehorse within the Eastern Metropolitan Region of Melbourne, Victoria.

It brings together a range of primary care agencies, GPs through Divisions of General Practice, and Metropolitan hospital services to improve service coordination, plan and evaluate together and undertake joint health promotion activities.

The mission of the IEPCP is to create effective partnerships between primary care providers, carers and consumers and the acute sector in order to achieve sustainable development of the primary care system and realise optimal outcomes for consumers.

The work of the IEPCP has been defined for September 2004 through to July 2006. Stakeholder meetings of the Partnership shaped the plan (from July to September 2004) that is now documented in the Community Health Plan 2004/06.

The Memorandum of Understanding (MOU)


The MOU is the agreement between all the member agencies and was approved at the first meeting of the new PCP. This document guides the work of the Partnership and covers:
  • Purpose and mission
  • Shared value system
  • Governance and organisational structure
  • Membership
  • Dispute resolution
  • Intellectual property and insurance
The MOU is to be reviewed annually and this current MOU is seen as a transitional agreement with a review that will occur in May 2005. All member agencies will be asked to participate in the review.

Governance


The Governance Committee for the IE PCP:

The IEPCP Governance Committee comprises members that provide partnership expertise and an individual sector/specialist agency view.

These are:
  • Community Health
  • Local Councils
  • General Practice Divisions
  • Metropolitan Health Services
  • Aged Care Assessment Service
  • Royal District Nursing Service


All of the above sectors have multiple agencies across the four municipalities. Each sector identifies their Governance Committee representative(s) for a 12-month period. Specialist agencies that are represented on the Governance Committee include:

  • The Migrant Information Centre
  • Uniting Care Community Options

Primary Care Partnership Committee Structure


The diagram below shows that the Governance committee is a sub group of the membership. Three committees have been established that are directly responsible to the Governance committee and are chaired by members of the Governance committee. These committees report directly to the Governance Committee.
The Governance committee reports to all members of the IE PCP through the six monthly forums.

The Manager is accountable to the Governance Committee for the implementation of the Annual Community Health Plan and reports on a frequent basis to the Governance Committee.

Staff employed will report to the Manager and the staff have the responsibility to work with and report on the progress of the reference groups that have been established to implement the Community Health Plan.

Staff


The core staff employed by the Inner East Primary Care Partnership includes the Manager, the Service Coordination Manager and the Health Promotion Project Cordinator.

All staff are full time and assisted by a part-time admin support worker. Other staff are employed from time to time depending on the special projects that are being run. For example an additional half time Service Coordinator position began in January 2005 to assist with the Boroondara service coordination work as well as the regional focus of service coordination for this Community Health Plan.
